Registration & Withdrawal


Registration Information

Registration will take place by appointment only.
Please call the school at 407-522-3400 to make an appointment.

» In order to register a student, a parent or guardian is required to complete the entire Registration 
Packet  paperwork  and  provide all documentation items listed below.

**Registration for either the IB or EPM Magnet Program must follow School Choice Magnet Program procedures. Information on applying to any Magnet Program can be found on the Magnet Application Process page.
     - All current 8th graders interested in any Magnet program must apply online at the School Choice website, OCPS Magnet Application link. This includes 8th graders currently enrolled in a middle school magnet program. 
     - All current 9th graders not in a magnet program and interested in applying to any Magnet program must apply online at the School Choice website, OCPS Magnet Application link.

  1.  Download blank registration packet and save to your computer with using Adobe Reader only.

  2.  Open saved document.

  3.  Fill in all the blanks.

  4.  Save completed registration packet with child’s First Name and Last name and print the documentation.
  5. Visit Student Services at Evans High School between 8:00a - 1:30p to complete registration. 

* Blank paper copies of the Registration Packets are available in the EHS front office for families without access to a printer.  


The following documentation is REQUIRED to register and MUST BE brought with you to your registration appointment

  1.  Completed registration packet (link above)

  2.  Birth certificate

  3.  Vaccination record on FL680 form

  4.  Proof of physical within last year

  5.  Academic history (transcript and/or last report card, withdrawal from prior school and most recent Test scores.)

  6.  Special education information (IEP, 504, etc.)

  7.  Government picture ID for registering parent (Driver’s license)

  8.  Verification of Guardianship (see registration packet)

  9.  Verification of Residence (see registration packet)

Withdrawal Information

All withdrawal documents must be submitted via email, fax, or in-person. Students must return all school owned property and items in order to be withdrawn

Email all withdrawal documents in ONE EMAIL to EHS Registrar, Fasha Jacobs. 

» FAX 
(1) Fax all withdrawal documents 
to 407-522-3458 for your child to be withdrawn AND,
(2) Send a confirmation email to EHS Registrar, Fasha Jacobs.
